Brethine (Terbutaline sulfate)

Brethine refers to tocolytic, antiallergic, bronchodilator. This drug selectively excites beta2-adrenoreceptors, increases adenylate cyclase activity, stabilizes cAMP, reduces the concentration of intracellular calcium, and relaxes smooth muscle. It relaxes the longitudinal bronchial muscles (bronchodilator effect), reduces the viscosity of the bronchial secretions, increases the motility of the ciliated epithelium (improves mucociliary Cl). It also brings the membranes of mast cells to the norm rate (anti-allergic effect). It suppresses the release of allergy mediators (serotonin, histamine, etc.). It relaxes uterine smooth muscle providing a tocolytic action.

Composition and release form

  • Tablets (2.5 mg);
  • Metered-dose aerosol for inhalation (1 dose – 250 mcg);
  • Powder for inhalation (1 dose – 500 mcg);
  • Injection solution (in 1 ml – 500 µg).

Indications

  • Bronchial asthma;
  • Asthmatic status;
  • Obstructive bronchitis;
  • Pulmonary emphysema;
  • Acute heart failure;
  • Fibrous cystitis;
  • Primary dysmenorrhea;
  • Threat of premature birth;
  • Fetal distress syndrome.

Dosage

  • Orally (tablets) to adults at a dose of 5 mg every 6 hours (in case of the development of expressed side effects – 2.5 mg 3 times a day);
  • Children over 12 years old – 2.5 mg 3 times a day.
  • The maximum daily dose is: for adults – 15 mg, and for children – 7.5 mg to the lateral deltoid region at a dose of 0.25 mg;
  • if there is no effect within 15–30 minutes, the injection is repeated at the same dose (the next injection is no earlier than after 4 hours).
  • Inhalation twice (with an interval of 60 seconds) every 4-6 hours.

Overdose

Symptoms: tremor, nervousness, impaired consciousness, convulsions, headache, sweating, tachycardia, arrhythmia, extrasystole, hypotension, nausea, vomiting, hyperglycemia, alternating with hypoglycemia.

Treatment: gastric lavage, the intake of activated carbon, the maintenance of vital functions.

Side effects

Brethine (Terbutaline sulfate) can cause the following side effects:

  • palpitations;
  • tachycardia;
  • tremor;
  • myalgia (pulling character);
  • anxiety;
  • sweating;
  • weakness;
  • headache;
  • dizziness;
  • mydriasis;
  • arrhythmogenicity;
  • decrease or increase in blood pressure;
  • nausea;
  • heartburn;
  • vomiting;
  • bladder atony;
  • allergic reactions;
  • paradoxical, discomfort or pain in the chest.

Contraindications

  • Hypersensitivity;
  • Thyrotoxicosis;
  • Decompensated heart defects;
  • Sinus tachycardia;
  • Extrasystole;
  • Premature detachment of the placenta;
  • Metrorrhagia;
  • Intrauterine infection;
  • Pyelonephritis during pregnancy;
  • Pregnancy (III trimester – for inhalation).

Take with caution if you have:

  • arterial hypertension;
  • pregnancy (I trimester);
  • ketoacidosis (administration of high doses);
  • history of epileptic seizures;
  • arrhythmia;
  • CHF;
  • diabetes mellitus;
  • pheochromocytoma.

Interactions

Not prescribed simultaneously with halogen-containing drugs for inhalation anesthesia, MAO inhibitors, beta-blockers, epinephrine, tricyclic antidepressants.

Pregnancy and lactation

Brethine is recommended for use from the 16th to the 37th week of pregnancy.
