What is Flovent?

Flovent is a glucocorticosteroid for topical use. The active substance is Fluticasone. The substance suppresses the proliferation of mast cells, eosinophils, lymphocytes, macrophages, neutrophils, reduces the production and release of inflammatory mediators and other biologically active substances (histamine, Pg, leukotrienes, cytokines). In recommended doses, it has a pronounced anti-inflammatory and antiallergic effect, which helps to reduce the symptoms, frequency and severity of exacerbations of diseases accompanied by airway obstruction (bronchial asthma, chronic bronchitis, emphysema). The systemic effect is minimal: in therapeutic doses, it has practically no effect on the hypothalamic-pituitary-adrenal system. The drug restores the patient’s response to bronchodilators, allowing to reduce the frequency of their use. The therapeutic effect after inhalation use begins within 24 hours, reaches a maximum within 1-2 weeks or more after the start of treatment and persists for several days after withdrawal. Indications for use include bronchial asthma (basic therapy), COPD.

Contraindications

Flovent is not prescribed for children under 4 years of age and also for patients with hypersensitivity to fluticasone. It is necessary to use the medication with caution if the patient has an infectious-inflammatory lesion of the mucous membrane and sinuses. Infectious diseases are not a contraindication to the use of the inhaler but they require a responsible approach to treatment. The drug is not recommended for use if the patient has recently undergone surgical treatment or trauma to the nose. Also, the treatment should be canceled if the patient has ulcerative lesions of the nasal mucosa. Fluticasone is not intended for use as part of a combination treatment with CYP3A4 isoenzyme inhibitors or other glucocorticosteroid drugs.

Side effects

Most often, Flovent is well tolerated by patients, but in rare cases, it can cause the following side effects: candidiasis of the oral mucosa and pharynx, hoarseness, paradoxical bronchospasm. With prolonged use in high doses, concomitant or previous use of GCS of systemic action in rare cases, there is a decrease in the function of the adrenal cortex, osteoporosis, growth retardation in children, cataracts, increased intraocular pressure. Extremely rare side effects include allergic reactions. Overdose can cause a temporary decrease in the function of the adrenal cortex. In acute overdose, emergency therapy is not required since the function of the adrenal cortex is restored within a few days. In chronic overdose, it is recommended to monitor the reserve function of the adrenal cortex. Treatment should be continued in doses sufficient to maintain a therapeutic effect.

Dosage and administration

Inhalation only. The drug should be used regularly, even in the absence of symptoms. To make an inhalation, you need to shake the inhaler, exhale the air from the lungs to the maximum, take a deep breath while pressing the buttons of the inhaler, hold your breath for as long as possible.

  • Dosage for adults: previously on bronchodilators alone: 88 mcg twice a day (the interval should be about 12 hours); the maximum dose is 880 mcg twice a day. Rinse mouth after inhalation. Return to the lowest effective dose after you achieve stability. Increase the dose if control is inadequate;
  • Dosage for children 4–11 yrs: 88 mcg twice a day (the interval should be about 12 hours). Rinse mouth after inhalation. Return to the lowest effective dose after you achieve stability. Increase the dose if control is inadequate.

Safety precautions

Treatment of bronchial asthma should be accompanied by a regular examination by a doctor. The therapeutic effect is provided by regular use of the drug. Adverse interactions with other drugs are unlikely. The simultaneous use of Flovent and inhibitors of the enzyme CYP3A4 (for example, ketoconazole, ritonavir, erythromycin) may increase the systemic effect of the glucocorticosteroid. This drug can be used to treat pregnant women only if the condition is life-threatening and when the attending physician has carefully weighed the benefit/harm ratio. Lactation must be suspended at the time of treatment. Fluticasone is intended for long-term treatment of bronchial asthma but not for the relief of seizures.

The drug is used with caution in liver cirrhosis, glaucoma, hypothyroidism, systemic infections (bacterial, fungal, parasitic, viral), osteoporosis, and pulmonary tuberculosis. The increased need for the use of inhaled short-acting β2-adrenoreceptor agonists indicates a worsening of the disease. The drug can exacerbate concomitant allergic diseases (for example, allergic rhinitis, eczema), which were previously suppressed by systemic drugs. In such situations, it is recommended to conduct symptomatic treatment with antihistamines and/or topical agents, including GCS for local use. The effect is reduced when the spray is cooled. The optimal effect is achieved after daily administration of Fluticasone for 3–6 months. If clinical improvement has not occurred, additional research is needed. Abrupt withdrawal of the drug is not recommended. Special care is taken when treating patients with pulmonary tuberculosis in both active and inactive forms. After you achieve a therapeutic effect, the dose of inhalation should be reduced to the minimum effective dose that allows you to control the course of the disease. When treating children, regular control of their growth dynamics is required, especially against the background of prolonged use of Fluticasone. Patients with COPD have an increased risk of pneumonia, so they should monitor their condition carefully careful and remember about the similarity of the clinical signs of exacerbation of the underlying disease and pneumonia.
